Once the Intern has been accepted the following is necessary: Both documents below are to be submitted with the student's background check to the address on the cover sheet. The background check is the responsibility of the student. All the information for the background check is located in the Affiliation Agreement All this information must be completed prior to the student's arrival at the hospital. All background checks should be sent to the Clearing House 2 months prior to the intern's start date (the AAS says 4 weeks but, we want to make sure everything is received in a timely manner). If the Clearing House does not receive the background check 2 months prior to the students start date, it will delay the start date and the student will not be able to begin the internship as scheduled. For additional information please see the AAS requirements for the background check. If you have any questions please feel free to contact Dominique Hafley @ dominique.hafley@uhhospitals.org or by phone at 216 844-7795. Background Check Cover Sheet
